This service will be conducted at the Purok Dos Multi-Purpose Building in Barangay Upper Bicutan on May 24, 2025, from 8:00 in the morning until 4:00 in the afternoon.
The initiative primarily targets those needing late birth registration.
Applicants need to obtain a Certificate of Live Birth form from the Mobile Birth Registration location or directly from the City Civil Registry office at Taguig City Hall.
After filling out the necessary details on the Certificate of Live Birth form (either typewritten or computerized), it must be signed.
On the reverse side, complete the requirements and sign the Affidavit for Delayed Registration of Birth. The completed form needs to be notarized.
Applicants must also obtain a negative certification from the Philippine Statistics Authority (PSA) as proof that no birth certificate has been filed for the applicant.
A certified true copy of the Marriage Certificate from the PSA or the Local Civil Registry Office is also required.
For parents who are not married, the Affidavit of Acknowledgement/Admission of Paternity found on the back of the Certificate of Live Birth Form and the Affidavit to Use the Surname of the Father (AUSF) need to be completed, signed, and notarized.
The following documents are required: the National ID of the parent(s), a valid ID of the parent(s), and two of the following documents that verify the applicant's parents' names, birth date, and place of birth:
a. Original Baptismal Certificate from the parish
b. Medical Certification or Immunization Card from the hospital, health center, or doctor
c. Certified True Copy of Form 137/School Records (for ages 7-17)
d. Certified True Copy of Voter's Registration (for ages 18 and above)
Individuals not registered within 30 days of birth are considered late birth registrations.
Children aged 7 to 17 who have not been registered must be accompanied by their parents when filing for late birth registration.
If the parents are married, only one parent needs to file. If the applicant is 18 years old or older, parents need not be present.
If the parents are not married, both mother and father are required to present at registration.
The father's presence is crucial during birth registration if he is not married to the child's mother, and vice versa, unless the parent in question is deceased, living abroad, or has abandoned the family.
Grandmothers or the mother's mother are also permitted to register, provided they have an Affidavit of Guardianship.
Even those not born in Taguig City can apply for late birth registration. They can still register, and the City Civil Registry office in Taguig will assist in coordinating with the local government of their birthplace. This is called Out-of-Town Registration.
Registration is free of charge, yet applicants must pay for requirements such as notarization, barangay clearance, and negative certification from the PSA.
For further inquiries, you may contact the City Civil Registry at the Ground Floor of the City Hall Annex Bldg., General Luna St., Barangay Tuktukan, Taguig City.
